⇒発行申請書をダウンロードし、記入をして学校へ提出してください。

　発行申請書が必要な場合は、学校へ連絡してください。

証明書が作成できましたら、学校から連絡します。印鑑(外国籍の方は在留カード)を持って受け

取りに来てください。

　郵送を希望する場合は、発行申請書・本人確認書類・切手を貼った返信用封筒を送付してください。

The school will contact you once thecertificate is created. Receive with your seal (for foreign nationals,residence card)

Please come to pick it up.

If you wish to send it by mail, please senda reply envelope with an issuance application, identity verification documents,and stamps.
